Please complete the following form. All applicants must include with their application, any photos/slides/video (not returnable), Glamour Lens Magazine will pay the $25.00 processing fee to Classmate USA.
RULES: High School or College Students, full or part-time, currently enrolled or planning to attend, submit application and materials by February 15. Applications received after this date will automatically be placed into the following years' search. Applicants under the age of 18, if selected, will require a parental release. We suggest you send in your application as early as possible. If you have a code #, please list it below.
